The only one that I could actually apply for a partner account was xxxgalleries4free...

XXXthumbs4free I got redirected to the main page of the site when I clicked the apply here link.

Also I tried the form on bravsfreeporn a couple of times and it worked fine so I'm not sure what went wrong there.
